Great thread, just wanted to add my $.02.
I've always heard abouth the "take 1 day on 1 day off" approach in regards to opiates but that's more for the recreational user. If you have the ability to do so that's probably a great short term plan
For chronic pain we all have run into the problem of running out or losing med (I left 1/2 a script in Canada and didn't realize til I was back in the states) and having to bite the bullet for a few days. At least in my experience that's the only time I turned to the black market.
It's a fine line , as is most thing with opiates. I remember a time where I said "the heck with it" , flushed my meds and started going to NA daily....worst 6 days of my life. I guess I didn't realize how far down the rabbit hole I had gone.

For pain killers, the only thing that has ever work for me was 1) never increasing the dosage, and when I say never I mean never, 2) only taking it 1 time per day (not before bed), and 3) also taking a weaker opiate like Kratom in the process.
Aside from that, I don't think you can control tolerance buildup and the incoming addiction from it.
Well, maybe taking a small dosage of naloxone with it all could help.
